I/My company hereby confirmed that I/My company have appointed Costgo International (HK) Limited as my/our insurance agent to handle all my/our insurance matters including placing/renewing insurance policy with immediate effect. I/My company understand the policy provisions and accept the above company to provide service.

旅遊支援服務免費提供予私家車綜合保險之投保人。如以公司名義投保，請提名一位（公司東主或公司員工）以享用此服務。

最後聲明

本人，作為提案車輛之擁有人，明白並聲明本人於本表格內所提供之所有資料及聲明均屬正確真實，並無遺漏。本人同意此投保表及聲明書將構成本人與 QBE 香港及上海保險有限公司（透過 歌詩高國際有限公司 安排）所訂立之保險合約之基礎，並由 QBE 香港及上海保險有限公司及歌詩高國際有限公司認可，如有任何虛報或漏報，將會使保單作廢。

I/We, the owner of the proposed vehicle, declare that to the best of my/our knowledge and belief the foregoing answers are true and complete in every respect. I/We agree that this Proposal and Declaration shall be the basis of and be deemed to be incorporated in the contract of insurance, including any renewal thereof, between me/us and QBE Hongkong & Shanghai Insurance Ltd., arranged through Costgo International (HK) Limited.
